Survey responses inform our research, and that research informs practice - shaping the services, accommodations, and supports that help children return to school and childcare after a brain injury or concussion.

Healthcare providers
We are looking for Healthcare providers in Texas to complete an online survey (about 10 minutes) about the types of services and accommodations they believe would be beneficial when a young child or student returns to childcare or school following a brain injury/concussion.
We will mail you a $20 gift card after you complete the online survey.
Educators and Childcare Providers
We are looking for Childcare providers (daycare, preschool, early intervention) or school staff (educator, administrator, school psychologist, school nurse, paraeducator) in Texas to complete an online survey (about 10 minutes) about services young children and students with brain injuries/concussions receive when they return to childcare or school after their injury.
We will send you a $10 gift card after you complete the online survey.
Parents and Primary Caregivers
We are looking for Primary caregivers of children aged 0–21 who sustained a brain injury/concussion in the past 3 years and live (or receive childcare/education or healthcare services) in Texas to complete an online survey (about 15 minutes) about their child’s brain injury/concussion.
We will mail you a $20 gift card after you complete the online survey.
Students (10-17)
We are looking for students aged 10-17 who sustained a brain injury/concussion in the past 3 years and live in Texas. Parents/Guardians will need to provide permission for their child to participate. If the parents/guardians provide permission, students will complete an online survey about their brain injury/concussion.
We will mail them a $20 gift card after they complete the online survey.
